IMPORTANT: In order for MDHSA to review and assign credits, we must receive a verified transcript from the former school(s). A verified transcript is one that is emailed directly from the school to MDHSA (diploma@masondixonhomeschoolers.org) OR mailed to MDHSA, 32 W. Main St. #5, Waynesboro, PA 17268. Failure to submit a transcript will result in the PSR form being rejected.
