Confirmation of Receipt of Paperwork
Use this form if you wish to receive confirmation of your mailed paperwork. A self-addressed stamped envelope must be included.
Annual Summary Form and Transcript (ASF/TX)
Due to continued misuse of this form, the Annual Summary and Transcript Form is watermarked and requires the use of the Diploma Program Participant password to access it.
NOTE: This version is for families only. MDHSA Diploma Program evaluators have a different password to access to both the printable version of the ASF/TX and an online version. If they use the online ASF/TX, families will automatically receive a copy of this form and an Annual Status Report when the student transcript is updated. Talk to your evaluator if you wish to use this option.
Diploma Request Form (DR)
After the 12th grade evaluation, submit this form along with the final paperwork to request a diploma.
Education Verification Requests
If you are an employer, military recruiting officer, or other institution wanting to submit an education verification request, please send your request along with a signed release form to diploma@masondixonhomeschoolers.org
Common App Transcript Request (CATR)
ONLY use this form for uploading to the student’s Common App account. For more information on the Common App and its services, please visit the Common App website.
Public/Private School Review Form (PSR)
Use this form if transferring from a public/private/charter/cyber school and you would like MDHSA to review the transfer records (fee charged) to determine what credits may be assigned.
Application for Prior Credits (PC)
Use this form to apply for credit for high school level work completed prior to 9th grade. Form must be submitted before or with the initial Diploma Program Registration Form.
Music Checklist – Use this form if your student will be applying for a Performance in Fine Arts diploma in music.
Foreign Language Study Checklist – Use this form if your student is using a non-textbook approach to study a foreign language. If applying for Academic Honors, this form must be used/submitted if a textbook is not used.
