Instructions for Emailing Yearly Paperwork

  • EMAIL your student’s yearly paperwork as an attachment to forms@masondixonhomeschoolers.org.
  • Only PDF documents will be accepted. If you have a photo of your document, please either find an appropriate app to convert the photo to a PDF or use the “Print to PDF” option on your printer. JPEGs or other images will not be accepted because the staff does not have time to convert images into PDFs especially during the evaluation season.
  • PDF’s must be clear and easy to read. All text should be visible on the document. Faded, blurry, or partial documents will not be accepted.
  • Make sure two sided documents have both the front and back as separate pages. 
  • Evaluations and the Annual Summary Form (ASF) and Transcript (TX) MUST have a clear Evaluator’s signature. NO EXCEPTIONS. The documents will not be accepted without a handwritten signature.
  • If possible, send only ONE file per yearly submission. If you send each page as a separate, individual attachment, it will greatly increase the time it takes to update your student’s file. To ensure that your student’s paperwork is processed as quickly as possible, please combine all pages into one PDF file.
  • If submitting more than one year, please group the paperwork for each grade together OR by combining like-paperwork together. See table below.
